Graduate Student Evaluation Forms
Annual Report
- Annual Progress Report: To be completed each February by advisor and student. This report is used by the Department to evaluate student progress in their program.
Doctoral Evaluation Forms
These forms are used in the Department to evaluate certain milestones in the Ph.D. program.
- Preliminary Exam Evaluation Form: To be completed by the relevant exam committee. A form must be submitted to the DGS after each area exam in the written component and after the oral exam in the oral component of the preliminary exam.
- Dissertation Topic Evaluation Form: To be completed by the dissertation committee upon the completion of the oral dissertation topic proposal; oral defense of the topic; and the written dissertation research outline.
- Dissertation Evaluation Form: To be completed by the dissertation commit- tee upon the completion of the dissertation and oral presentation and defense of the dissertation.
Master’s Evaluation Forms
These forms are used in the Department to evaluate the capstone experience in the M.A. program.
- Comprehensive Exam Evaluation Form: To be completed by the exam committee. A form must be submitted to the DGS after each area exam and after each part of comprehensive exam.
- Project Evaluation Form: To be completed by the project supervisor upon completion of the written report and oral presentation of the project.
- Thesis Evaluation Form: To be completed by the thesis committee upon completion of the thesis and oral presentation and defense of the thesis.
